Job Description
Title: Physical Therapist

Schedule: Shift: 5x8 Days

Assignment Details:
- Contract Length: 13 weeks

Requirements:
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the state of practice.
- Experience in skilled nursing facility (SNF) or geriatric rehabilitation preferred.

Roles & Responsibilities:
- Evaluate residents and develop individualized treatment plans to improve mobility, functional independence, and quality of life.
- Deliver skilled physical therapy services in compliance with physician orders, facility policies, and applicable regulations.
- Monitor residents' progress and adjust care plans as necessary to achieve therapy goals.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides.
- Document treatment sessions, progress notes, and billing information accurately and in a timely manner.
