Day Shift 5 x 8s, M - F 8am to 5pm. Must have LVN certification. At least 1 year of experience. This will be a clinical intake coordinator for home health processing and scheduling referrals. Maintain accurate intake records in CRM and ensure data integrity. Set up and manage intake pipelines to track lead progress and milestones. Meet and report on key intake metrics and targets to leadership. Uphold data privacy and confidentiality in handling sensitive information. Screen and triage inquiries to determine eligibility and next steps. Provide accurate timelines and communicate expectations to applicants and stakeholders. Collaborate with teams to schedule follow-ups and ensure timely responses. Identify process bottlenecks and suggest improvements for faster case handling. Participate in ongoing training to stay current with process changes and tools. Assist in onboarding new hires by coordinating initial intake setup and access.
