Remote Care Manager: Empower Independence & Outcomes

City of Lincoln

The City of Lincoln is seeking a Care Manager to work with individuals with intellectual and developmental disabilities in Passaic, NJ. This role involves collaboration with families and various support providers to assist clients in achieving personal outcomes. The position is primarily remote with occasional in-person visits required in Manhattan and Rockland. Candidates need a bachelor's degree or registered nurse license with relevant experience.
