Geriatric Home Care Nurse — Clinical Leader

On Lok PACE is seeking a Home Care Registered Nurse to provide direct skilled nursing services to participants enrolled in our PACE program, under the supervision of the Home Care Manager. You will develop and adjust home care plans and supervise geriatric aides and LVNs to ensure high-quality care.
