Pride Health is hiring a Physical Therapist (PT) - Home Healthtosupport our client's healthcarefacility in Sebring, FL. Job Details Schedule: Monday-Friday, 8:00 AM - 4:30 PM Location: Sebring, FL 33870 Float / Coverage Area: Highlands County and Hardee County, Florida Job Type: Local contract Contract Length: 13 weeks Pay Range: $52.00 to $57.00/hour* Mileage Reimbursement Rate (if applicable): $0.51/mile *Pay offered is based on experience,expertise, credentialing, and education. Duties Perform comprehensive physical therapy evaluations and assessments in the home setting. Complete OASIS assessments, reassessments, recertifications, and discharge evaluations as required. Develop and implement individualized Plans of Care (POC) based on physician orders, patient goals, and clinical findings. Provide therapeutic interventions to improve strength, mobility, balance, endurance, safety, and functional independence. Monitor patient progress and modify treatment plans to achieve optimal outcomes. Educate patients, caregivers, and family members on therapeutic exercises, fall prevention, disease management, and home safety practices. Recommend and train patients in the proper use of assistive devices and adaptive equipment. Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary healthcare team members. Maintain accurate and timely documentation within Homecare Homebase (HCHB). Meet productivity expectations while delivering exceptional patient-centered care. Requirements Active and unrestricted Florida Physical Therapist (PT) License. Minimum 1 year of Home Health experience. Experience completing OASIS assessments. Proficiency with Homecare Homebase (HCHB).
