Therapist Physical Therapist

Job Summary:

Provides comprehensive physical therapy services to patients in an inpatient and swing bed setting, focusing on restoring mobility, function, and independence. Assesses patient conditions, develops individualized treatment plans, and educates patients and caregivers throughout the recovery process.

Responsibilities:

• 
  Perform patient assessments using reliable and valid examination methods to determine diagnoses and treatment needs.
  
  
• 
  Develop and adjust personalized care plans incorporating patient goals, clinical outcomes, and discharge strategies.
  
  
• 
  Implement therapeutic interventions including manual therapy, modalities, and patient transfers, ensuring safety and efficacy.
  
  
• 
  Educate patients, families, and healthcare team members on treatment procedures, progress, and home care strategies.
  
  
• 
  Supervise support staff according to legal and ethical standards, providing regular feedback on care delivery.
  
  
• 
  Document patient progress, treatment details, and billing information accurately in EMR systems.
  
  

Qualifications:

• 
  Hold a Bachelor's Degree in Physical Therapy from an accredited program.
  
  
• 
  Possess an active Illinois Physical Therapist (PT) license.
  
  
• 
  Maintain certification in BLS through the American Heart Association.
  
  
• 
  Be capable of performing patient transfers and body mechanics safely.
  
  
• 
  New graduates are encouraged to apply, with relevant clinical training preferred.
