Medical Social Worker - Henderson County

Social Worker Reports To: Director of Patient Family Services Status: Salary (exempt) Employment: Full Time (FT) Shift: Days - Monday to Friday Location: Henderson County Position Summary: Provide social work services and support that ensures the patient and their family or caregiver receive psychosocial services according to Four Seasons Compassion for Life mission statement, policies, and procedures. Essential Duties & Responsibilities Perform initial and ongoing psychosocial assessments of patients/family/caregiver with a holistic perspective. Implement and evaluate a plan of care for each assigned unit. Serve as liaison to community health, welfare, and social agencies and make referrals. Contribute to patient and family counseling and education regarding family dynamics and coping methods. Assist client/family/caregiver in determining health care and grief and loss goals. Address advocacy needs for patients, families, and the organization. Organize and facilitate the discharge of patients. Actively participate in IDG team meetings for development and evaluation of the patient’s plan of care. Evaluate financial needs and make appropriate interventions for financial assistance. Assist families/caregivers in making final arrangements prior to or at time of patient’s death. Knowledge, Skills & Abilities Knowledge of Federal, State, and Local regulations and Medicare Conditions of Participation. Knowledge of HIPAA privacy rules. Knowledge of electronic medical record systems. Expertise in discharge planning. Knowledge of Hospice Services including Hospice Medicare Benefit. Basic data entry skills. Excellent organizational skills. Excellent interpersonal skills. Excellent written and oral communication skills. Ability to work under pressure to meet deadlines. Ability to take initiative and utilize innovative techniques. Ability to interact with various departments. Ability to work flexible hours when needed. Team player to coordinate projects. Ability to plan, prioritize, and coordinate multiple projects. Minimum Qualifications Education: Bachelor’s in Social Work (BSW) or Master’s in Social Work (MSW). Experience: 1–3 years experience within a healthcare setting. Licensures & Certifications (if applicable) Valid NC Clinical Social Worker license (LCSW) preferred. Valid driver’s license and proof of automobile insurance. Summary of Physical Requirements Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling. Arm/wrist flexion/extension. Hand dexterity to include gripping, pushing/pulling, and writing. Other Duties Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values. Perform duties in compliance with Medicare Conditions of Participation. Participate in coordination of the patient’s hospice care in accordance with professional standards. Practice within the scope of license, certification or registration, where applicable. Develop and maintain professional relationships with all team members involved in client care. Function in various care environments, including home, skilled nursing facilities, assisted living, etc. Evaluate bereavement/pre-bereavement needs and develop a bereavement plan of care. Assist patients and caregivers in understanding levels of care and financial implications. Obtain necessary information from third‑party providers for payment. Facilitate internal referrals. Provide 24‑hour on‑call psychosocial services. Conduct pre‑admission consults and/or facilitate consent forms. Attend staff and mandatory in‑services meetings. Participate in ongoing education and update professional skills. Practice safety and infection control methods. Perform other similar duties as assigned.
