Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: 8-Hour Days Hours per Shift: 37.5 hours per week Experience: Minimum 2 years License: Client License required Certifications: BLS Certification required Must-Have: Flu vaccine required, no exceptions Description: The medical management department is seeking an RN case manager to provide a range of direct and indirect services for patients and families. This includes the identification of psychosocial problems and their resolutions. The RN case manager collaborates with the patient, family, and interdisciplinary team to effect safe and timely discharge and treatment plans. Responsibilities include: Completing initial and concurrent insurance reviews. Completing PRI for patients requiring skilled nursing facility placement. Making psychosocial assessments utilizing all available data, including interviews with the patient and family and collaborating with interdisciplinary associates. Ensuring continuity of care through the development, implementation, and validation of discharge and treatment plans, in a timely manner, and in collaboration with the patient, family, and interdisciplinary team. Functioning independently while utilizing supervision to identify any problems in expediting discharge and treatment plans. Ensuring confidentiality in accordance with hospital and social services policies. Developing ongoing linkages and networking relationships. Participating in patient-focused conferences and in internal and external conferences. Demonstrating leadership skills by utilizing psychosocial expertise to assist with team building and discharge/treatment planning. Demonstrating conflict resolution and psychosocial education to interdisciplinary staff when appropriate. Participating in departmental programs, including quality improvement, statistical reporting, continuing education, staff meetings, and other related assigned duties. Demonstrating knowledge of legal and ethical issues and principles related to end-of-life care, including informed consent, the right to refuse treatment, and advance directives. Demonstrating skills and knowledge related to the identification, reporting, and appropriate intervention for victims of violence, abuse, and neglect. Demonstrating appropriate interventions and timely and safe discharge and treatment plans through sound psychosocial assessments. Demonstrating organization of workload with appropriate priorities based on medical plans, patient needs, and departmental policies and standards. Seeking help and resources to avoid unnecessary delays in service. Demonstrating proactive advocacy, mediating, and negotiating skills with patients, families, associates, and the community. Qualifications include: RN with BSN preferred. SCREEN certification preferred. Demonstrates an understanding of the impact of medical, social, and environmental factors on patient and family systems.
