Job Title Nurse Practitioner Float - MC Location Oak Park, IL Job Description Provide care across 13 clinics in Chicago, Evanston, Brookfield, and Oak Park. Work collaboratively with a dedicated clinical care team to deliver evidence-based care. Perform assessments, diagnoses, treatment, management of health problems, and health counseling. Requirements include prior primary care or urgent care experience. Submit BLS and license documentation; BLS from ARC or AHA required. Complete candidate questionnaire including licensure status and experience in primary care. Critical Information Average 30-35 hours per week; Monday - Friday from 8:30 AM to 7:30 PM, alternating weekends. On-site as needed; this is an in-person, patient-facing role. Education and Licenses Needed Current PA license in the state of application; must be in good standing. High School Diploma or GED required.
