Duties and Responsibilities Assess patient’s health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. Advise patients on health maintenance and disease prevention or provide case management. Liaise with physicians and other team members to plan and evaluate patient care plans. Modify patient treatment plans as indicated by patients' responses and conditions. Guide and supervise health care personnel like LPNs and home health aides. Manage a caseload of patients, ensuring all patients receive the required care as per the established guidelines. Maintain accurate, detailed reports and records of patient conditions and treatments. Monitor, record, and report symptoms and changes in patients' conditions in a timely manner. Order, interpret, and evaluate diagnostic tests to identify and assess patients' conditions. Coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. Direct and coordinate activities of nurses, assistants, specialists, therapists, and other medical staff. Ensure compliance with laws and regulations in health care. Participate in educational opportunities to update knowledge and skills. Requirements Must hold a valid RN license in Oregon. Preferred experience in home health care settings. Expertise in managing a caseload of home health patients. Ability to coordinate with LPNs and other members of the home health team. Proven ability to provide compassionate and effective care to patients. Strong organizational and communication skills. Must be able to work efficiently without direct supervision. Demonstrated capability in making clinical judgments in a high-paced, dynamic environment. Knowledge of computer operations and proficiency in the use of basic word processing, data entry and clinical information systems. Adept at handling and resolving challenging clinical situations. Commitment to continuing education and professional development. Flexible with work hours, able to handle weekend or evening assignments if necessary, and available to respond to emergencies.
