Outpatient Physical Therapist

Provides patient care assessment and treatment. Performs initial evaluations. Assesses patients' motor and development skills to determine their deficits. Evaluates patients' physical and mental abilities and analyzes data for a realistic rehabilitation plan of care. Reviews physician referrals and patient medical records to determine diagnoses and necessary treatment. Records prognosis, treatment, response, and progress in patient medical records. Prepares written materials related to patient care, including evaluations and plans of care. Maintains patient records confidentially. Facilitates patient mobility skills using safe patient handling equipment. Manages equipment utilized in treatments. Adapts and modifies environments, equipment, and materials to promote functional mobility and address patients' deficits. Documents and identifies progress and plans for reevaluation. Evaluates the effects of treatment and adjusts plans to maximize benefits. Facilitates social adjustment and prevents physical or mental disabilities. Engages in comprehensive age-appropriate evaluations. Sets quantifiable and realistic goals for patient progress. Utilizes advanced judgment and skills for implementing treatment plans. Ensures timely communication with physicians and up-to-date documentation. Accomplishes patient mobility using safe patient handling techniques and equipment. Maintains cumulative physical therapy records. Facilitates excellent customer service and takes initiative to resolve client concerns. Educates patients and families about treatment procedures. Instructs patients and families to facilitate skills in physical functioning and health maintenance. Trains caregivers on how to meet patient needs during and after therapy. Promotes activities to help patients learn mobility skills within their capabilities. Implements programs and activities to assist patients in learning mobility skills and adjusting to limitations. Recommends changes in patient work or living environments based on needs and capabilities. Monitors and holds coworkers accountable. Models appropriate communication and behavior. Collaborates with other healthcare team members in a multidisciplinary approach. Assists in the orientation of new employees. Mentors students and other personnel as assigned. Convenes with patients and medical practitioners to plan and assess intervention programs. Maintains clinical competencies through professional growth opportunities. Participates in continuing education to ensure consistency with best practices. Builds positive relationships with healthcare team members and the community. Assists in departmental promotional efforts.
