Travel Nurse RN - Case Management - $1,993 per week

LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Chesapeake, Virginia.

MUST HAVE recent experience in an adult acute hospital setting, with discharge planning!

5+ years of recent acute care case management experience (hospital setting)

Census 15-25 for Care Management (typically indicates they are familiar with the model)
• 4/5 or 5/5 with all Care Management functions (including CMS regulations)
• Epic experience REQUIRED
• Experienced traveler (unable to consider first assignment at this time unless years of experience are extensive)

5x8 Days 8am-430pm, weekdays, some weekends, holidays, no overtime without supervisor approval

Discharge Planning, ortho/neuro surgical unit, ability to handle fast paced unit with multiple discharges and admissions each day

Must have UR experience

BLS required (must be via AHA), CCM or ACM Preferred
