Pharmacist PACE and Hospice of Cincinnati
TriHealth is actively recruiting for a Pharmacist to join the PACE of Cincinnati PACE Center located in Norwood Ohio. Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. This unique opportunity is for a full-time position, M-F 11:30 AM- 8:00 PM to service in a position to support PACE of Cincinnati and Hospice of Cincinnati as a clinical and dispensing pharmacist.
The PACE Clinical Pharmacist is responsible for completing comprehensive initial medication management assessments for new PACE participants and medication management reassessments on existing PACE participants. For Hospice of Cincinnati, responsible for the filling and dispensing of medication orders including the proper handling, usage, and storage of drugs in the pharmacy. Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy. Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug related information and literature. Plans and organizes work schedule of supportive personnel and pharmacists, as required.

Work Schedule: M-F 11:30am-8:00pm
Job Requirements: Bachelor's Degree Bachelor's of Science or Pharm D with current license in State. (Required) 1 - 2 years One year of experience with a frail, elderly, or long-term care population. Experience caring for frail older adults' physical, mental, emotional, and social needs. Demonstrates the ability to work in an interdisciplinary team setting effectively. Basic Life Support (BLS) Required
