Master Of Social Work

Key Responsibilities: Conduct assessments toidentify clients' social, emotional,and behavioral needs. Develop and monitor individualized service/care plans. Provide supportive counselingand crisis interventionas appropriate. Coordinate referrals to healthcare, mental health, housing, financial assistance, and community resources. Advocate for clientsand help them navigate available services. Maintain accurate and confidential client recordsand documentation. Collaborate with healthcare providers, casemanagers, schools, families, and other professionals. Participate in caseconferences and multidisciplinaryteam meetings. Monitor client progressand make recommendations for additional services. Follow organizational policiesand applicable federal, state, and local regulations. Typical Qualifications: Master's degree in Social Work (MSW) from an accredited program. LMSW or LCSW preferred/required depending on the position. Strong communication, assessment, documentation, and interpersonal skills. Experience working with individuals, families, or vulnerable populations preferred. Knowledge of community resources and social service systems. Ability to maintain confidentiality and handle sensitive situations professionally. #J-18808-Ljbffr
