Position: Public Health Nurse (PHN) 

Location: Toppenish, WA 98948 

Shift: Monday through Friday, 8:00 AM – 5:00 PM

Contract Length: 1 year with the option to extend

Estimated Start Date: Pending contract award and completion of the onboarding process 

Compensation:

• Regular Pay Rate: $40 hour
• Weekly Stipend for travelers: $1,246
• Health & Welfare (benefit per hour worked) : $5.09
• Estimated Weekly Gross (based on 40 hours): $3,049
• Please keep in mind that payroll is processed on a biweekly basis. 

Benefits: 10 days PTO, Accrued sick days, 11 Paid Holidays 

Job Description:

Provide public health nursing services focused on communicable disease surveillance, case management, community outreach, and health promotion activities. 

Requirements:

• Bachelor’s degree in Nursing from an accredited nursing program
• Current, unrestricted Registered Nurse license in a U.S. jurisdiction
• Current BLS certification
• At least one (1) year of experience in public health nursing or community outreach
• Demonstrated knowledge of communicable disease control and contact tracing
• Ability to coordinate effectively with public health agencies and interdisciplinary teams 

Responsibilities:

• Conduct case management, outreach, and contact tracing for communicable diseases
• Perform directly observed therapy (DOT) as required
• Coordinate with local, state, and federal public health agencies
• Review laboratory results and provide patient education
• Administer medications and immunizations
• Conduct field visits and community follow-up
• Deliver public health education sessions
• Document activities in the EHR or approved public health systems
• Travel to outreach events using a contractor or personal vehicle
