Part-Time Weekend Baylor RN Registered Nurse (Home Health)

Empath Home Health, a member of Empath Health, is seeking a compassionate part-time Weekend Baylor RN Registered Nurse (Home Health) to work with our Home Health team to deliver individualized care to patients in their homes throughout North Hillsborough and East Pasco counties (North Tampa, Dade City, Land O'Lakes, and surrounding areas).

Locations: Home health visits in North Hillsborough and East Pasco counties (including North Tampa, Lutz, New Tampa, Tampa Palms, Wesley Chapel, Land O' Lakes, Dade City, San Antonio, and surrounding areas).

Schedule: This is a part-time Weekend Baylor role requiring full weekends. Saturday and Sunday (20 hours per week).

What You'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience
• Home Health RN experience required
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
