Job Summary

Physical Therapist

8:30-5pm with a weekend rotation of every 3rd weekend. Day shift, 8 hr shifts for 5 days no on-call Licenses: PT licens

Responsibilities

• Provide evidence-based physical therapy assessments and treatment plans.

• Perform hands-on physical therapy sessions and document progress.

• Adhere to safety, compliance, and professional ethics in all activities.

• Set measurable patient outcomes and track progress against targets.

• Collaborate with multidisciplinary team to optimize rehabilitation plans.

• Utilize evidence-based protocols to ensure high-quality care and consistency.

• Maintain accurate, timely documentation in electronic health records.

• Educate patients on exercises, injury prevention, and home programs.

• Identify opportunities for professional development and pursue certifications.

• Engage in pipeline and territory planning to optimize patient referrals.

• Demonstrate accountability for individual outcomes within a team environment.

Experience/Qualifications

• Licensed Physical Therapist with active state license and clinical competencies.

• Evidence-based assessment, treatment planning, and outcome measurement skills.

• Hands-on therapy delivery and accurate progress documentation in EHR.

• Safety, compliance, and professional ethics adherence in all sessions.

• Ability to set measurable patient outcomes and monitor progress against targets.

• Strong collaboration with multidisciplinary teams to optimize rehab plans.

• Maintain high-quality care via evidence-based protocols and standardized procedures.

• Patient and caregiver education on exercises, injury prevention, and home programs.

• Proactive pursuit of professional development and certifications to advance skills.

• Strong accountability for individual outcomes within a performance-driven team.
