Contract | 13-Week Assignment | Onsite | Skilled Nursing Facility (SNF)
Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to restore or improve physical function and mobility
- Develop and implement individualized physical therapy treatment plans based on patient needs and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and their families on therapeutic exercises and home care strategies
Required Qualifications
- Active Physical Therapist (PT) license in the state of Missouri or eligibility to obtain licensure
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing facility (SNF) setting preferred
- Ability to fulfill a 35-hour weekly guarantee
Preferred Experience / Skills
- Prior experience in a SNF or long-term care environment
- Familiarity with Medicare documentation and compliance requirements
