Geriatrician

Older adults need a physician who can hold complexity without losing the person. In this role you will set the medical plan for patients living with multiple conditions, cognitive change, and shifting goals-and make sure the rest of the care system follows that plan. Your work will be measured in safer medications, fewer avoidable crises, and more days lived the way patients choose.
Performance Objectives
  • Complete comprehensive geriatric assessments and care plans that reduce avoidable hospital use and support the least restrictive, safest setting.
  • Reconcile and simplify high-risk medication regimens so treatment matches function, cognition, and what the patient values.
  • Direct interdisciplinary care with nursing, therapy, pharmacy, and social work to cut falls, delirium, delayed discharges, and failed transitions.
  • Lead goals-of-care and advance-care planning conversations, and document decisions the full team can act on.
  • Improve geriatric quality results-transitions, falls, delirium, and preventive care-by building workflows the team can sustain.
  • Coach colleagues and trainees so geriatric standards hold when you are not in the room.
Environment & Resources

You will practice with a dedicated geriatric team, pharmacy support, therapy, care management, and an electronic record suited to complex chronic care. Coordination staff manage scheduling and authorizations so visit time stays clinical. You report to the geriatrics medical director and help set protocols, not only individual treatment plans.
Essential Qualifications
  • MD or DO, board-certified or board-eligible in geriatric medicine, or in internal or family medicine with substantial geriatric practice.
  • Active license and ability to obtain privileges in the practice setting.
  • Proven results caring for complex older adults in outpatient, inpatient, or post-acute care.
  • Skill in polypharmacy review, cognitive and functional assessment, and goals-of-care discussions.
  • Experience leading interdisciplinary teams and meeting quality and documentation standards.
