Exciting Opportunity for a Physical Therapist!

Job Description:
- Join a dynamic team focused on enhancing patients' physical functioning!
- Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care.
- Develop and implement tailored care plans following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practices to guide clinical interventions.
- Administer a range of rehabilitation procedures, including:
- Manual techniques
- Ambulation exercises
- Therapeutic exercises and modalities
- Use of supportive and assistive devices
- Wound care
- Assess patient progress and adapt treatments as necessary, ensuring compliance with regulatory and clinical requirements.
- Evaluate treatment outcomes for effectiveness and modify services based on evaluations.
- Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
- Communicate effectively with supervisors and health team members about patient progress, challenges, and treatment plans.
- Prepare comprehensive discharge summaries that meet all regulatory and clinical requirements.
- Document all required information in the electronic documentation system, maintaining patient records as per company policies.
- Accurately record treatment minutes for services provided.
- Participate in essential meetings, including:
- Patient care conferences
- Utilization review meetings
- Rehabilitation meetings
- Engage in required facility and company trainings, in-services, and conferences.
- Manage the procurement of any necessary Durable Medical Equipment (DME) or supplies per regulatory and clinical standards.
- Supervise and support physical therapist assistants, rehabilitation aides, and/or students.
- Adhere strictly to APTA and State practice acts, codes of conduct, and ethics.

Submission Requirements:
- Minimum of 1 year of PT experience is required.
- New Graduates welcome, as long as they have Level II experience (not just Level I observation).
- Experience in Long-Term Care (LTC) or Rehabilitation is required.
- Massachusetts State License is required.
