This role includes routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and close collaboration with facility partners. Qualifications: Current NP or PA licensure in the state of practice Previous professional patient care experience Experience in geriatrics, family medicine, or internal medicine preferred.
