Job Description

The Float Nurse acts as a fill in for nurse coverage for all Longleaf offices, responsible for the overall management of all nursing care for assigned patients.

As a Float Nurse, you will:

• Use health assessment data to support Plan of Care
• Triage calls and assist patient/primary caregiver with problem resolution. Counsel the patient and family in meeting nursing and related needs
• Make visits for symptom management and support as needed
• Modify patient treatment plans as indicated by patients' responses and conditions
• Maintain current, accurate, detailed reports in accordance with policy
• May monitor, record and report symptoms and changes in patients' condition to the IDG
• Provide health care instructions to the patient as appropriate per assessment and plan
• Obtain and receive physician orders as required
• Pronounce patients
• Other duties as assigned

Requirements

Current license to practice as a registered professional nurse in Georgia.

Current and continued CPR certification.

At least one or two years of recent home care experience; hospice experience preferred.

Good verbal and written communication and problem-solving skills.

Responsible valid driver's license. Reliable, insured, automobile for making home visits required.
