Location: Barre, VT 05641
Job Type: Contract
Duration: 20 Weeks
Shift: Day Shift
Schedule: Monday – Friday
Guaranteed Hours: 36 Hours Weekly
Start Date: Typical start date is within approximately 2 weeks after offer acceptance and completion of onboarding/credentialing requirements.

Experience Requirements

Minimum 1 year of RN experience required
Hospice case management experience required
Home hospice care experience required
Rural travel experience preferred
Experience with McKesson/Netsmart EMR preferred

License & Certification Requirements

Active Vermont RN License required
BLS/BCLS certification required

Job Summary

Seeking an experienced Hospice RN Case Manager for a home health and hospice assignment in Vermont. The RN will provide hospice care services in patient home settings, coordinate patient care plans, manage case documentation, and collaborate with interdisciplinary teams to support quality patient outcomes.

Monday–Friday schedule
Holiday coverage required
Mileage reimbursement provided for travel between patients at IRS rate
No 1099 accepted
EMR system: McKesson
Facility prefers candidates with minimal or no RTO requests during the contract
