Registered Nurse - Hospice Case Manager

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. Schedule: Monday-Friday 8:00am-5:00pm Location: Anderson, Franklin, Owen, and Woodford Counties Position Summary The Hospice RN Case Managerdeliverscompassionate and comprehensive health care services to Hospice patients in a home care setting,orwhereverthe patient calls home.Home care settingsmayconsistofa private residence, skilled nursing facility, personal care home, and/or other interim livingarrangements.The Hospice RN Case Manager’sgoal for excellentpatient care standardsbegins with active listening to create and implement a holistic plan of care based on the patient’s goals andpreferences.The Hospice RN Case Manager serves as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers as they navigate Hospice Care.The plan of care will include managing symptoms throughpharmacologic and non-pharmacologic interventions foroptimalcomfort, functionality, and safety. Frequent monitoring and follow-up arerequiredto evaluate the efficacy of treatments, plans of care, and to offer emotional support to both the patient and the caregiver. A Registered Nurse in this position will havestrong communicationskills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. Position Qualifications Graduateof an accredited school of registered nursing. Currently licensed as a Registered NursethroughtheKentucky Board of Nursingor through a participating CompactStateBoard of Nursing. Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered. Demonstrates strong clinical nursing skills and the ability tomaintainprofessionalism during emotionally challenging situations. Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity. Adheres to accepted professional, ethical, and legal standards of nursing practice. Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying. Maintains current CPR certification. Must be a licensed driver with an automobile in good working order, that isinsuredin accordance withstateminimumrequirements.
