Hospice RN Case Manager: Coordinate Compassionate End-of-Life Care
Hugh Chatham Memorial Hospital
Advocate Health is seeking a Hospice RN Case Manager to coordinate care for terminally ill patients in hospice settings. You will educate patients and families, lead the IDG, and ensure appropriate care planning and documentation. This full-time role in Illinois offers 40 hours per week, a $3,000 sign-on bonus, and a comprehensive benefits package.
