Intake Coordinator

Every patient we serve starts with intake. When a referral comes in, someone has to move fast, gather the right information, coordinate with the clinical team, and make sure nothing gets dropped between the hospital and the home. That's this role, and it matters more than most people realize. At Interim HealthCare of Oklahoma City, we've been locally owned and part of this community since 1999. Our staff volunteers with Meals on Wheels, we host monthly Veteran-to-Veteran coffees through our hospice program, and we show up for this city the same way we show up for our patients. When you work here, you're part of something that goes beyond the office. What you'll do- Receive and process incoming referrals from hospitals, physicians, and other referral sources
- Gather and verify patient information, insurance, and authorization requirements
- Coordinate with clinical staff to ensure a smooth transition from referral to first visit
- Schedule the initial patient visit and communicate details to the care team
- Maintain accurate documentation throughout the intake process
- Serve as a professional and responsive point of contact for referral sources

- Clear, professional communication with referral sources, patients, and clinical staff
- Someone who understands the urgency of intake and treats every referral like it matters - because it does
