Certified Occupational Therapy Assistant (COTA) - SNF Setting We are seeking a compassionate and skilled Certified Occupational Therapy Assistant (COTA) to join our team at Astoria Healthcare Center in Sylmar, California. As a COTA at our skilled nursing facility (SNF), you will play a vital role in helping our residents regain their independence and achieve their goals. About the Role As a COTA, you will work under the supervision of a licensed occupational therapist to design and implement individualized therapy plans for our residents. Your expertise will focus on improving mobility, strength, and functional abilities, as well as promoting cognitive and emotional well-being. You will also collaborate with other healthcare professionals to ensure comprehensive care and service delivery. Responsibilities Assist occupational therapists in the development and implementation of individualized therapy plans Conduct daily assessments and progress reports to track resident improvements Implement evidence-based occupational therapy interventions to promote resident independence Provide education and training to residents, families, and caregivers on post-hospital care and home safety Document all therapy sessions and progress in accordance with company policies and regulatory requirements Participate in interprofessional team conferences and develop discharge plans as needed Maintain accurate and up-to-date records and files Requirements Current certification as a Certified Occupational Therapy Assistant (COTA) Completed an associate's degree program in occupational therapy assistance Strong knowledge of occupational therapy principles, techniques, and best practices Excellent communication and interpersonal skills Ability to work in a fast-paced environment with multiple priorities Familiarity with electronic medical records (EMRs) and therapy documentation software What We Offer Join our team of dedicated and compassionate healthcare professionals who care about our residents and each other.
