Healthcare Referral & Outreach Liaison
Midland Care Connection Inc
Midland Care Connection is seeking a Clinical Liaison to serve as a bridge between our not-for-profit organization and hospitals, clinics, and care coordinators. You will promote Midland Care’s hospice, home health, and PACE programs, educate referral sources about enrollment criteria, and support admissions with timely information.
