Coordinate the necessary evaluations and assessments to identify each youth's needs utilizing assessment tools. Provide transportation as appropriate and necessary to reach identified goals. Maintain and model appropriate interpersonal relationships. Advocate for client and family and perform tasks necessary to remove barriers to necessary, consistent care. Meet the utilization requirement identified by your program and supervisor. Participate actively in supervision. Reviews any problems and concerns with supervisor to ensure effective and efficient program operations. Qualifications:Minimum of a High School Diploma plus 3 years of experience in a related field, or a Bachelor's degree in a relevant field. Excellent written and communication skills.Ability to work flexible hours, including some evenings and weekends. Initiative and ability to work independently. A willingness and comfort level in providing community-based services to a diverse population of clients.
