Coordinate patient care and manage discharge planning in a clinic environment that values collaboration and patient outcomes. Assess patient needs, develop care plans, and work with multidisciplinary teams to ensure safe transitions. To qualify, you need a current Maryland RN license or compact eligibility, graduation from an accredited nursing program, and at least 2 years of recent experience in patient care delivery or clinical health care. Experience with electronic medical record (EMR) systems and knowledge of healthcare regulations are required. Leadership experience is recommended. Recommended skills include strong communication, critical thinking, and problem-solving abilities.
