Hospice RN Case Manager

RN Case Manager - Hospice Responsibilities Provide ongoing assessment and nursing care to patients in homes and health care facilities. Ensure accurate evaluation of patient needs and develop and implement comprehensive care plan. Follow current nursing professional standards of practice and agency-approved protocols for hospice. Provide quality plan and symptom management for patient comfort. Communicate with other health care providers about changes in condition and patient needs. Assess patient and caregiver needs and provide education when necessary. Commit to high-quality customer service for internal and external customers. Oversee implementation of the patient's Plan of Care and evaluate effectiveness on an ongoing basis. Prepare for IDG by reviewing the Plan of Care, reconciling medications, and projecting patient/family needs. Audit and maintain accurate patient records using laptop computers and paper/chart documentation in accordance with policies, federal, state, and local guidelines. Participate in quality improvement projects as needed. Maintain knowledge of regulatory agencies, accrediting bodies, and third-party payers' requirements. Perform all other duties as assigned. Qualifications Current, unencumbered license to practice as a Registered Nurse (RN) in Ohio. One year of experience in acute care or medical-surgical nursing. Experience in hospice care (preferred). Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet. Proficiency with electronic medical records (EMR) (preferred). Other Requirements Must read, write, speak, and understand English. Must possess a valid driver's license and an acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards. Working Conditions and Special Requirements Sitting – up to 4 hours/day. Standing – up to 8 hours/day. Walking – up to 6 hours/day. Lifting, transferring, pushing or pulling patients or equipment/supplies – up to 50 pounds. Driving – up to 6 hours/day. Work weekends, evenings, and holidays (required). On-call availability – 24/7 for emergencies.Risk: exposure to bloodborne diseases, falls, burns, infectious waste, etc. #J-18808-Ljbffr
