As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care , you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. You will report to our Associate Medical Director. Your Role & Responsibilities You will provide comprehensive care to seniors following clinical standards. You will collaborate daily with your care team to ensure coordinated, high-quality care. You will manage referrals, hospital/SNF coordination, DME, and home health services. You will document care accurately and efficiently with support from quality-based coders. #J-18808-Ljbffr
