We’re hiring a Director of Patient Care Services – a compassionate and experienced clinical leader who oversees hospice care delivery, supports interdisciplinary teams, and ensures regulatory compliance. What You’ll Be Doing: * Serve as a role model and resource for nursing staff and interdisciplinary team members * Support staff in problem-solving and determining appropriate clinical interventions * Collaborate with the Administrator on hiring, orientation, retention, and performance management

• Adhere to budget requirements for labor, pharmacy, DME, and medical supplies
• Coordinate patient care with RN Case Managers and attending physicians
• Review and manage patients receiving higher levels of care (respite,

inpatient, continuous care)

• Oversee clinical and management responsibilities for assigned teams
• Serve as liaison between Administrator, clinical staff, patients, families,

and community partners Qualifications: * Graduate of an accredited school of nursing with a bachelor’s degree or higher in nursing or a related health field. * Current, unencumbered Registered Nurse (RN) license in the state of employment. * Active CPR certification required; certification in Hospice and Palliative Nursing preferred. * Must meet one of the following: * Bachelor’s degree or higher in nursing or a related health field and at least 2 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years * At least 4 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years * Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable.

• Valid driver’s license with a clean driving record.
• Reliable transportation and willingness to travel as needed.
• Ability to lead bereavement support groups and make sound clinical decisions.
• Flexibility to work outside regular hours based on patient needs.

Anticipated pay range

$140,000—$152,000 USD
