We are seeking a professional and compassionate  Registered Nurse (RN)to conduct initial and ongoing assessments of clients receiving in-home care services.

The RN Assessment Nurse is responsible for evaluating each client’s health status, functional abilities, personal care needs, safety concerns, and overall care requirements. The RN will assist the agency in developing an appropriate individualized plan of care and determining whether the client’s needs can be safely and appropriately met by the agency.

This position is primarily assessment and care-planning focused and is ideal for an RN interested in flexible or PRN home-care work.

Essential Duties and Responsibilities

The Registered Nurse will:

• Conduct comprehensive  initial assessments of incoming clients in the client’s home or other approved setting.
• Review the client’s medical history, diagnoses, medications, allergies, physician information, and current health conditions.
• Assess the client’s ability to perform  Activities of Daily Living (ADLs) , including bathing, dressing, grooming, toileting, eating, transferring, mobility, and other personal-care needs.
• Evaluate  Instrumental Activities of Daily Living (IADLs) , including meal preparation, housekeeping, laundry, shopping, transportation, and other applicable support needs.
• Assess mobility, fall risk, skin condition, nutritional concerns, cognitive status, communication abilities, and environmental or home-safety concerns.
• Identify changes in condition, potential health risks, and circumstances that may require referral to a physician or other healthcare professional.
• Determine the appropriate level and frequency of home-care services based on the client’s assessed needs and authorized services.
• Develop, review, and update individualized  plans of care/service plans based on assessment findings.
• Provide caregivers and agency staff with appropriate instructions regarding the client’s care needs and limitations.
• Conduct reassessments and supervisory visits as required by agency policy, applicable regulations, payer requirements, or changes in the client’s condition.
• Communicate significant changes in a client’s condition to appropriate agency personnel, physicians, family members, responsible parties, or other healthcare professionals as appropriate.
• Document assessments, reassessments, nursing notes, care recommendations, and other required information accurately and promptly.
• Maintain complete and confidential client records in accordance with agency policies and applicable privacy requirements.
• Participate in care coordination with clients, family members, caregivers, physicians, case managers, and other members of the client’s care team when appropriate.
• Report suspected abuse, neglect, exploitation, unsafe living conditions, or other significant concerns according to agency policy and applicable reporting requirements.
• Assist agency management with clinical questions and client-care concerns within the RN’s professional scope of practice.
• Maintain professional boundaries and provide respectful, client-centered care.
• Perform other nursing assessment, supervisory, and care-coordination duties consistent with the RN’s scope of practice and agency policies.

Qualifications

The successful candidate must:

• Possess a current and unrestricted  Registered Nurse (RN) license authorizing practice in South Carolina.
• Be a graduate of an accredited nursing program.
• Maintain current CPR/BLS certification if required by the agency or applicable program.
• Have strong assessment, documentation, communication, and organizational skills.
• Be able to work independently while maintaining effective communication with agency management.
• Demonstrate knowledge of elderly, disabled, special-needs, and medically vulnerable populations.
• Be able to identify changes in client condition and appropriately communicate or escalate concerns.
• Maintain client confidentiality and professional standards at all times.
• Successfully complete required background checks, screenings, and other employment requirements.

Preferred Qualifications:

• Previous home health, home care, hospice, long-term care, geriatric, community health, or case-management experience.
• Experience conducting in-home nursing assessments.
• Experience developing or reviewing individualized plans of care.
• Familiarity with Medicaid, waiver, or other community-based home-care programs.

Physical Requirements

The RN must be able to:

• Travel to clients’ residences and other designated service locations.
• Safely enter and navigate residential environments.
• Sit, stand, walk, bend, and perform routine physical activities necessary to conduct client assessments.
• Observe and assess clients in their home environment.
• Use a computer, tablet, or electronic documentation system as required.

Schedule

This position may be structured as  PRN, part-time, or per-assessment , depending on agency needs and client volume. The RN must be available to complete assessments and required documentation within established agency and payer timeframes.
