As a VITAS Hospice Admissions Nurse, you'll be the compassionate bridge between hospital or community care and the comfort of hospice. You'll guide patients and families through one of life's most important transitions with clarity, empathy, and confidence. This isn't just a job—it's a calling. Schedule: 4x10's with weekend rotation and shift starting at 10am OR option for Thursday-Sunday schedule.What You'll DoConduct comprehensive clinical assessments to determine hospice eligibilityEducate patients and families about hospice services and Medicare benefitsCollaborate with physicians, case managers, and interdisciplinary teamsProvide emotional support and clinical guidance during the decision-making processInitiate and complete the admission process, including documentation and coordinationWhat We're Looking ForRegistered Nurse (RN) with a valid state licenseMinimum 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred)Strong clinical assessment and critical thinking skillsCompassionate communicator with excellent interpersonal skillsActive listener who builds trust and rapportConfident in guiding others through uncertainty, addressing concerns with empathyReliable professional with strong follow-through and autonomyPatient-centered advocate committed to dignity and comfortWhy You'll StayMission-Driven Work.
