Duke Health Integrated Practice comprises more than 110 primary and specialty outpatient clinics, extending the reach of the Duke Health mission across the state of North Carolina.

We are seeking a highly motivated high-risk breast cancer Advanced Practice Provider in the Duke Breast Risk Assessment Clinic located in Raleigh, NC and Cary, NC to work in collaboration with attending physicians to provide direct patient care services, collect and document data, conduct diagnostic/therapeutic procedures, and order/schedule imaging studies and professional consultations. This position will play a key role in identifying and managing patients at an elevated risk for breast cancer. Responsibilities include evidence-based risk assessment, screening, prevention, patient education, and care coordination designed to promote early detection and reduce breast cancer risk. Primary hours will be Monday through Friday, day shift, with flexible hours to meet the needs of the patients.

Job duties and responsibilities include:

• Provide direct patient care services for new and return patients, both in person and virtually, including triage, physical exams, collecting and documenting data, and coordinating care.
• Develop individualized screening and risk reduction plans for high-risk breast patients. Continue to monitor and follow patients for changes in risk status, clinical changes, and genetic updates.
• Counsel and educate patients on breast cancer risk and management recommendations.
• Interpret genetic test results and communicate findings to patients and care teams.
• Maintain working knowledge of hereditary cancer guidelines and integrate evidence-based guidelines into clinical practice.
• Conduct diagnostic and therapeutic procedures (such as skin punch biopsies).
• Order imaging, laboratory studies, and professional consultations.
• Conduct pre-authorizations for recommended studies, as ordered by the APP and/or other team members.
• Prescribe appropriate interventions and medications such as risk reducing medications.
• Follow-up test results and coordinate ongoing care, including but not limited to imaging and pathology results.
• Conduct timely e-communication consults from referring providers.
• Communicate verbally and in writing all pertinent patient information to collaborating physicians, referring providers, nursing staff, patient resource managers, and other relevant healthcare providers.
• Provide timely documentation in the electronic medical record of all relevant data including results of tests; laboratory results; patients' condition and response to therapies/interventions; communications with colleagues.
• Patient care responsibility will be completed in an out-patient clinic setting and will not involve in-patient, after hours call, or operating room time.
• Participate in educational experiences for trainees (students, residents, fellows, etc.) and orientation sessions for other team members.
• Seek out and participate in continuing education opportunities through case conferences, reading, attending seminars/webinars/conferences, etc., to enhance knowledge in breast cancer risk.
• Demonstrate professionalism in conduct, speech, appearance, work habits, customer service, communication style, critical thinking skills, resource utilization, and problem resolution.

Job eligibility requirements include:

• Education: Nurse Practitioners: Work requires completion of an accredited Nurse Practitioner program, with a master's degree in nursing. Physician Assistants: Work requires completion of an accredited Physician Assistant program. A bachelor's degree required; master's degree strongly preferred.
• Certification: BLS required. Additional training and/or certification favored, such as APP certification through ASBrS, completion of the City of Hope Intensive Course in Genomic Cancer Risk Assessment, and CGRA (Cancer Genetics and Risk Assessment) certification through NCoBC
• License: Nurse Practitioners: Must have current or compact RN licensure & NP approval to practice in the state of North Carolina. National certification as a NP required. Physician Assistants: Must be licensed in the state of NC and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants.
• Experience: APP experience preferred. Experience in a high-risk breast clinic or cancer genetics preferred. Experiences in a breast surgery clinic and/or hereditary cancer clinic favored.

Job hours are Monday-Friday, day shift, with flexibility to meet clinic needs.
