Physical Therapist - Travel Contract

Active California Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Home health experience required. Experience with HCHB (Homecare HomeBase) required. Productivity expectation: ability to meet approximately 5 productivity points per day (can be combination of follow-up visits and Start of Care visits). Previous experience managing territory assignments or caseloads across multiple teams is preferred. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Days, 8-hour shifts, Monday through Friday. Typical Schedule: Monday to Friday, 8:00 AM to 5:00 PM. Assignment Length: 12 weeks. On-call or weekend requirements: None.
