Position Summary Program Coordinator, LES Boston Job ID: 345 Location: West Roxbury, MA Shift: Monday-Thursday 8am - 4pm, Friday 7:30am - 3:30pm Salary: $61,655 Lifeworks is a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaging and meaningful lives. Responsibilities Promote optimal health for individuals through routine, preventive, and emergency health care and clinical services and ensure that work environments are safe. Supervise and perform case‑management responsibilities, including preparation, implementation, and evaluation of Individual Support Plans for employees funded by the Department of Developmental Services, Massachusetts Rehabilitation Commission, School Departments, etc., and work closely with the DDS Case Management Team Service Coordinators. Serve as the program Safety Officer and implement safety plans and programs. Encourage regular communication and cooperation between staff of all LES programs to coordinate rehabilitation and production efforts and maintain effective communication with residential providers and families regarding activities and supported employment placements. Coordinate scheduling of daily and weekly staff and employee work assignments for all individual, enclave supported employment placements and Life Enhancement activities; coordinate with Transportation Department for weekly transportation schedule. Coordinate evaluation and intake of referrals with leadership team. Provide coverage support as necessary for various sites, placements, activities, and outings in the community. Provide training, support, and follow‑up to individual and group placements. Maintain accurate records for individuals served for program evaluation, billing, etc. Coordinate accurate payroll records for all individuals and employees, job coaches, and relief staff. Assist leadership team in long‑term and short‑term planning, development, organization, and evaluation of services and the daily operation of the program. Implement Quality Enhancement standards within the program. Coordinate scheduling of all relief staff for the program. Perform other duties and/or assignments to work/program locations as needed. Requirements Experience working with individuals with ID/DD. Bachelor’s degree from a four‑year college or university and one to two years of related experience. Must be able to pass fingerprint background check, CORI, DPPC, and driving background checks. Valid driver’s license required. Excellent verbal and written communication skills.
