Job Summary:

Provide professional nursing care to patients in long-term care or skilled nursing facilities. Collaborate with healthcare teams to deliver comprehensive patient support in a residential setting.

Responsibilities:

• 
  Assess and monitor patients health conditions regularly.

  Administer medications and treatments as prescribed.
  
  
• 
  Document patient information accurately and maintain detailed records.
  
  
• 
  Communicate effectively with patients, families, and healthcare staff.
  
  
• 
  Adhere to safety protocols and institutional policies.

  Ensure compliance with all vaccination and identification requirements.
  
  

Qualifications:

• 
  Current RN license in the state of practice.
  
  
• 
  Proven experience in LTC or SNF settings preferred.
  
  
• 
  Ability to provide compassionate and patient-centered care.

  Strong communication and interpersonal skills.
  
  
• 
  Successful completion of applicable immunization requirements, including COVID-19 vaccination.

  Ability to wear identification badges and follow safety guidelines diligently.
