Jesse Helms - Skilled Nursing Facility Located on campus at Atrium Health Union in Monroe, NC Full Time | 36 Hours Per Week | Pay Range : $35.50 – $53.25 This position is sign‑on bonus eligible. Nurses with over one year of experience will be cross‑trained as backup for charge duties. Shift: 7 p.m. – 7 a.m. with alternating weekends. Essential Functions Responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation to meet prescribed plan of care and MD orders. Collects data pertinent to the healthcare consumer from patient assessments, telephonic contacts, and EMR information. Analyzes data to determine issues and resources for effective resolution, identifies high‑risk patients, and coordinates case conferencing. Prioritizes multi‑issue situations based on patient health, safety, and impact to service delivery. Identifies barriers to resolution (e.g., language, literacy), and follows up with appropriate resources or leadership. Develops and provides oversight of a plan of care that is effectively communicated and supports quality outcomes and satisfaction. Implements the plan and directs coordination of a multidisciplinary clinical team, supervising LPNs and/or HHAs to promote health and safe outcomes. Performs ordered treatments, therapies, and procedures in accordance with state regulations and agency policies. Evaluates progress toward attainment of patient‑centered goals. Physical Requirements Walking, standing, sitting, lifting (up to 35 lbs), reaching, stooping, bending, pushing, pulling. Must speak English clearly and have intact vision, hearing, hearing, smell, touch, and finger dexterity. Requires physical abilities testing. Education, Experience & Certifications Graduate from an accredited School of Nursing (BSN preferred). BLS required. Current RN license or temporary RN license or National License Compact‑approved license.
