Coordinate patient care as a team leader practicingPatient-Centered Medical Home principles Minimum Qualifications Education: Certification or graduation from a four(4) year college or university Licensure & Certifications: Current KY RNlicense Job Duties 65% - Monitor patient vitals; detect changes in symptoms,health, and/or pain.
