Occupational Therapist (OT) - SNF / LTC

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team. Responsibilities Provide direct patient care to assess their medical condition, functional capabilities, limitations, and restrictions, and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients. Document and record the patient's condition. Educate patients and families in an appropriate occupational therapy method. Provide skilled occupational therapy services / interventions in accordance with physician orders. Assess patient needs, plan for, evaluate and modify care to meet goals of occupational therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduated from an accredited school. Current state license as an Occupational Therapist / OT or proactively in the process of obtaining a state license. Current CPR certification. Experience in providing occupational therapy services in a skilled nursing or long term care setting (experience or newly graduated).
