We are seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will provide patient assessments and diagnoses. This role requires a strong commitment to patient well-being. The Nurse Practitioner will work collaboratively within a team and collaborate with physicians, nurses, therapists, and families. Duties

This role requires a strong commitment to patient well-being. The Nurse Practitioner will work collaboratively within a team and collaborate with physicians, nurses, therapists, and families. Duties
- Conduct comprehensive assessments of patients to determine their healthcare needs.
- Offer ongoing support and education to patients and their families.
- Collaborate with interdisciplinary teams to ensure excellent patient care.
- Document patient interactions accurately in the electronic health record system.
- Engage in continuous learning and professional development to stay current with best practices in medicine.
- Proficiency in EMR/EHR systems such as Epic or Athena health; telehealth experience is highly desirable.
- Excellent communication skills for effective patient assessment, family education, and interdisciplinary collaboration.
