Job Details Schedule: 8:00 am to 4:00 pm Coverage Areas: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells and Whitley Counties Location: Fort Wayne IN. Job Type: Contract Contract Length: 3 months with possibility of extension Pay Range: $52/hr to $56/hr. Pay offered is based on experience, expertise, credentialing, and education. Duties Perform Start of Care (SOC) visits, comprehensive evaluations, and ongoing assessments for home health patients. Develop and implement individualized physical therapy treatment plans based on patient needs and goals. Provide skilled physical therapy interventions to improve mobility, strength, balance, safety, and functional independence. Educate patients and caregivers on exercises, safety techniques, and home programs.
