RN Case Manager

Make it happen at Hopkins What you will do: In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. The Case Manager manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify/ eliminate barriers to the delivery of clinical services with the patient, family and care team. Develops a sustainable, safe transition plan appropriate to the patients’ needs and resources in collaboration with the patient/family and interdisciplinary care team. Master of Science in Nursing (MSN) preferred. Minimum three to seven years of acute nursing experience required; minimum of one-year in case management, and/or discharge planning required. Accredited Case Manager i.e. – Registered Nurse (ACM – RN) / Certified Case Manager (CCM) required Licensed to practice as a Registered Nurse in the District of Columbia. Salary Range: Minimum 43.25/hour - Maximum 67.04/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority. The Hospital reserves the right to modify employee schedules as needed.
