Advanced Practice Provider — Primary CareAre you a Nurse Practitioner or Physician Assistant looking for a primary care role where you'll build real relationships with your patients — not just see them once in a walk-in? Thundermist Health Center is looking for Advanced Practice Providers to join our primary care teams across Rhode Island. You'll manage your own patient panel, deliver continuity-focused care including preventive, acute, and chronic disease management, and work within an integrated care model alongside physicians, behavioral health clinicians, care managers, and community health workers. This is not a Convenient Care or walk-in role — it's primary care with a panel, a team, and a community that depends on you. Thundermist is a multi-site FQHC serving over 65,000 patients, and we're committed to delivering care regardless of insurance status, immigration status, or ability to pay.What You'll DoClinical Care & Panel ManagementProvide primary medical care to an assigned patient panel including scheduled visits, acute care, chronic disease management, and preventive carePerform comprehensive health histories, physical examinations, and diagnostic assessments; order, interpret, and follow up on labs, imaging, and testingPrescribe medications, including controlled substances, within scope of practiceProvide anticipatory guidance and patient education, including pediatric well-child guidance where applicable to specialtyCoordinate referred services including specialty care, pharmacy, and ancillary servicesTriage patient concerns and determine appropriate level of care, escalation, or transferTeam-Based Care & CollaborationCollaborate with physicians, other APPs, nurses, MAs, behavioral health clinicians, care managers, community health workers, and social work staffSupport warm hand-offs and integrated behavioral health workflows within primary carePartner with care management for high-risk and complex patientsCoordinate with community agencies and social service partners to address social determinants of healthSupervise and precept NP, PA, and medical students as assignedQuality, Documentation & ComplianceComplete clinical documentation in the EHR (eClinicalWorks) accurately and within same-day charting requirementsSupport HRSA UDS reporting and value-based care performance through consistent care gap closure, screening completion, and preventive care deliveryParticipate in peer chart review, quality improvement initiatives, and clinical care improvement effortsMaintain practice consistent with HRSA, OSHA, HIPAA, CLIA, and applicable state and federal regulationsMaintain credentialing, licensure, national certification, DEA registration, and BLS/ACLS/PALS/NRP as applicableWhat We're Looking ForYou're a provider who chose primary care because you want to know your patients by name, manage their care over time, and be part of a team that wraps around the whole person. You're clinically sharp, compassionate, and motivated by Thundermist's mission.Master's degree from an accredited NP program with Rhode Island APRN licensure, OR Master's from an ARC-PA accredited PA program with Rhode Island PA licensureNational certification in Family Medicine, Internal Medicine, Women's Health, or Pediatrics (ANCC, AANP, or NCCPA)Current DEA registration and Rhode Island Controlled Substance RegistrationCurrent BLS certification; ACLS, PALS, NRP as applicable to specialtyEligibility to enroll with Medicare, Medicaid, and commercial payers (current CAQH profile required)Working knowledge of electronic health records (eClinicalWorks preferred)Preferred:Prior experience in an FQHC, community health center, or safety-net primary care settingBilingual (Spanish/English or Portuguese/English) strongly preferredExperience with team-based care models, integrated behavioral health, and value-based careExperience with panel management, quality metric performance, and preventive care outreachEligibility for NHSC or Rhode Island state loan repayment programsCompensation & Provider SupportCompetitive compensation and benefits including: malpractice coverage through the Federal Tort Claims Act; health, dental, and vision insurance; retirement plan with employer match; generous PTO and CME allowance; eligibility for state and federal loan repayment programs. Typical schedule is Monday through Friday during clinic hours, with some evening clinic sessions to support patient access.
