Registered Nurse - Home Health

This role is suited for a Registered Nurse who values individualized patient care in a home health setting. You will provide one-on-one nursing services to patients in their homes across Bergen, Passaic, and parts of Morris County, supporting improved outcomes in a comfortable environment.

Day shift, Monday through Friday from 8:30 AM to 5:00 PM. Required rotation: every 4th weekend (Saturday and Sunday). Holiday requirement: 2 per year (1 winter, 1 summer).

Provide skilled nursing care including assessment, planning, implementation, and evaluation. Follow physician orders and ensure visit frequency aligns with care plans. Complete accurate and timely clinical documentation, including patient progress. Conduct comprehensive assessments, medication reconciliation, and care plan updates. Collaborate with physicians and interdisciplinary teams to coordinate care. Educate patients and families using tailored teaching approaches. Monitor patient condition and report changes to appropriate stakeholders. Ensure compliance with payer requirements and authorization processes. Supervise home health aides and complete required supervisory visits. Maintain productivity expectations and manage assignments independently. Stay current with clinical best practices and apply them in patient care. Perform additional duties as needed in a dynamic care environment.

Associate's Degree in Nursing or Nursing Diploma. Minimum 6 months of RN experience or 1 year of LPN experience (home care or hospice preferred). Active New Jersey Registered Nurse license. AHA Basic Life Support (BLS) certification. Basic computer proficiency.

Bachelor of Science in Nursing (BSN). IV Certification. Previous home health or hospice experience.

Ostomy care. Foley catheter management. G-tube care. Wound care and wound vac management. Patient assessment and care planning. Clinical documentation and compliance.
