Please consider joining a team committed to building brighter futures for all! Applies culturally responsive, trauma‑informed, and ethical practices. Demonstrates reliability, time management, and accountability for outcomes. Duties / Responsibilities: Conduct comprehensive intakes/discharges (as needed), ongoing assessments, and structured interviews to identify participant needs, strengths, and goals. Assist participants in obtaining identification and documentation, completing applications, and escorting participants to community appointments when necessary. Provide crisis intervention, safety planning, and emergency coordination as needed. Occasionally, travel throughout NYC using public transportation for home visits, appointments, and community‑based services. Maintain accurate, timely, and compliant documentation in AWARDS within 72 hours of all participant contact. Ensure all documentation follows agency templates and procedures and includes required signatures. Maintain organized, confidential, and audit‑ready participant files.
