RN Intermediate Care

Job Summary:

The Registered Nurse (RN) for the Intermediate Care Day Shift provides skilled nursing care in a 42-bed unit divided between medical-telemetry and intermediate care. The RN is responsible for managing patients with complex medical conditions such as pneumonia, COPD exacerbation, diabetic ketoacidosis, sepsis, and respiratory failure. This role requires proficiency in caring for patients with tracheostomies, ventilators, high-flow nasal cannula, cardiac drips, BiPAP, and telemetry monitoring. The RN collaborates with interdisciplinary teams to deliver comprehensive care while maintaining appropriate nurse-to-patient ratios. Responsibilities:

• Provide direct skilled nursing care to patients in intermediate care and medical-telemetry settings.
• Perform patient assessments, monitor vital signs, and manage complex medical conditions including respiratory and cardiac support.
• Administer medications and treatments per physician orders and protocols.
• Manage ventilators, tracheostomies, high-flow nasal cannula, cardiac drips, and BiPAP therapy.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Maintain accurate and timely documentation of patient status and interventions.
• Float to other units as needed, providing care on both medical-telemetry and intermediate care sides of the floor.
• Ensure patient safety and adherence to hospital policies and procedures.

Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of 1-2 years of nursing experience in a hospital setting, preferably in medical-telemetry or intermediate care.
• Current ACLS and BLS certifications.
• Experience with tracheostomy care, ventilators, high-flow nasal cannula, cardiac drips, BiPAP, and telemetry monitoring.
• Strong clinical assessment and critical thinking skills.
• Ability to work effectively in a fast-paced, acute care environment.

Preferred Skills & Certifications:

• Prior experience in medical intermediate care or step-down units.
• Familiarity with care of patients with complex respiratory and cardiac conditions.
• Additional certifications related to critical care nursing.

Special Considerations:

• Must be willing to float between medical-telemetry and intermediate care units as needed.
• Work location requires compliance with Massachusetts staffing and temporary nursing service agency licensing.
• Must have experience managing patients requiring advanced respiratory support and cardiac monitoring.
• Candidate must be located outside a 90-mile radius from the facility.

Scheduling:

• Day shift schedule on the intermediate care unit.
• Shift assignments may include floating between units based on staffing needs.
