21st Century is seeking a Full Time Physical Therapist Case Manager in Elk Grove/Sacramento area to lead home health patient plans and coordinate care with physicians. You will operate with autonomy, managing a flexible schedule and using company devices. Responsibilities include home evaluations, mobility assistance, and creating detailed treatment plans, while collaborating with PTAs and interdisciplinary teams.
